  • This required a comprehensive review of the patient record in order to abstract medical, surgical, ancillary, demographic, social, and administrative data to ensure complete data capture.
  • Codes were selected and assigned from the current versions of the ICD-9-CM, CPT, HCPCS, and DSM; in determining justification for code assignment, the patient record was proficiently searched to ensure proper documentation.
  • The knowledge required included advanced medical terminology, anatomy and physiology, disease processes, treatment modalities, diagnostic tests, medications, procedures, and the principles and practices of health services in addition to the organizational structure to ensure proper code selection.
  • When necessary, I assisted facility staff with documentation requirements in order to accurately reflect the patient care provided and provided technical support in the areas of policy and regulation, and reported incorrect documentation or codes notated in the patient record.
