  • Under the general supervision of PATHS' Behavioral Health Program Director, the clinician will provide integrated behavioral health services as part of patients' care team alongside medical providers.
  • These outpatient services will include individual and group sessions to address a wide range of behavioral health issues, including substance abuse.
  • Specific duties include: Provides individual counseling and crisis intervention to adults who are experiencing psychological or adjustment difficulties.
  • Provides group-counseling programs.
  • Meets with staff of partner agencies in order to exchange information, provide education and offer consultation re: treatment of clinic patients with mental health issues.
  • Provides clinical evaluations to determine mental status and substance use disorders, and treatability of outpatient individuals.
  • Facilitates linkage with crisis/emergency services when needed.
  • Provides general behavioral health case management to outpatient individuals.
  • Responds to partner agencies requests for clinical input, assessment, and education on behavioral health problems and issues as these may affect their individuals.
  • Provides general consultation to community agencies, which includes providing training, education to area groups, parents, teachers, and other community interest groups who desire improved skills in working with children, adolescents, adults, and families.
  • Maintains individual records, routine correspondence, and daily statistical forms within the time frames set by the supervisor.Maintains confidentiality and security of Protected Health Information (PHI) as outlined in the Confidentiality Agreement.
