Provide positive team enhancing suggestions on time management with assessment scheduling.
Train new employees on tasks to assist in learning their new employment.
Successfully work as a team to ensure there is equal amount of support with each team member.
Manage time in order to complete assessments, submit paperwork, and enroll on appropriate waivers.

Document all contacts with and relating to consumer in state mandated time
Submit paperwork within state mandated time frame.
Arrive at scheduled assessment on time.
Provide excellent customer service to consumer and family.
Return calls within 24 hours of receiving them.
Offer backup support to assessment team.
Educate callers on appropriate programs that meet their needs.
Complete questionnaire to assist with determining what caller's needs are.
Inform callers of correct information regarding different Ohio Home and Community Based Waivers, what would qualify them, and how to apply.
Refer callers to different information sources with questions needing further clarification.
Coordinate assessor schedules with appropriate locations per time frames.
Assist assessors while in the field with locating scheduled assessments.
Provide information to nursing facility representatives who are submitting state mandated paperwork for admissions.
Educate nursing facility representatives, hospice social workers, and waiver case managers on Ohio Administrative Code.
Review Pre Admission Screens and Level of Cares submitted for nursing facility admissions and change of payer sources.
Manage caseload within senior community on Home and Community Based waiver.
Review needs of each consumer.
Schedule quarterly phone calls, home visits every 6 months, and annual reassessments for waiver eligibility.
Return telephone contacts within 24 hour period.
Increase/decrease service plan as needed.
Provide individualized care and plan management to each consumer.
Coordinate with other agencies involved (Hospice, Medicare Home Health, Behavioral Health, Adult Protective Services, etc) in plan of care.
Complete monthly review on consumers with high costs of care.
Refer to outside agencies for emergency and routine services.
Educate consumers on healthcare management.
Document contacts with consumers within state mandated time frames.
Submit reports on at risk activities such as falls, theft and alleged abuse to state monitoring program.
Meet with supervisor on cases that require further supervision.
Provide excellent customer service to consumers on waiver services.
