At the beginning of my nursing career, I worked at Martin Health Systems for 20+ years. I cared for patients in an acute care setting on a busy, fast-paced twenty-six bed medical/surgical unit. I worked day shift for the first ten years and then switched to night shift for the remaining years I was there. Phases of care included pre-op, post-op, medical management, wound care, and all other aspects of generalized nursing in addition to being a relief charge nurse. I was also a full-time charge nurse on a twenty-four bed unit in the past with job responsibilities including but not limited to nurse/patient assignments, budget, payroll, staff scheduling and other various leadership duties. Expertise in EPIC computer system and McKesson medication delivery system. I am a knowledgeable, caring, hard working and service-oriented registered nurse.
The past three years of my professional career were spent as a home health marketer/registered nurse (November of 2016 to August of 2018) and then as an intake coordinator/registered nurse (September of 2018 to mid-October of 2019) for Activa, a Medicare home health agency. I was out in the community, encouraging my doctors and other facilities to use my company for home health services, including nursing, physical therapy, occupational therapy, speech therapy, and medical social work. I have an incredible desire to help people in need of medical services and stay in contact with the referring entities at all times during the care of their patients. I was truly passionate about being a home health marketer and the fact that I could still use my nursing license to also be of great assistance to my company and my accounts when necessary. But then I decided to take an internal/office position to learn another aspect/role of the home health profession with the same company. As the only intake coordinator in this area for Activa Home Health, including Martin, St. Lucie, Okeechobee, and Indian River counties, I was responsible for processing all the referrals. This included running the patient's Medicare number through a database to ensure patients had traditional Medicare, to inputting the patient in a specific home health database used by Activa. Then I would send out a detailed email notification to those involved in the patient's care. I worked closely with the entire referral team, including the home health liaisons and other clinical members of the team.

Intake referral coordinator for all of Indian, Martin, Okeechobee, and St. Lucie counties from September of 2018 through mid-October of 2019. I was responsible for processing all Medicare home health referrals, starting with running patient's through the Medicare database to assure enrolled in traditional Medicare plan, to inputting patient in a specific database called Kinnser, and sending out a detailed email on each patient. The email was sent to the Activa case managers, coders, billers, administration (including the CEO, director of professional services, home health liasion the referral was brought in by/for, and any other employee of Activa that needed to be included). I was also responsible for obtaining and processing referrals sent in via fax/email from hospitals, nursing homes, assisted living facilities, and skilled nursing facilities. I was the sole intake coordinator in the above listed counties and also developed relationships/rapport with case managers at facilities and worked closely with the home health liasions, marketing directors, directors of professional services, and two teams of schedulers and case managers who were also responsible for the referrals. I also was responsible for a daily tracking board of active patients admitted to hospitals, rehabs while on service with Activa and pending referrals as well. This process involved calling the facilities daily to check if patients still admitted and getting discharge updates.
Marketing/sales representative for a Medicare home health agency in Martin and St. Lucie counties from November of 2016 through August of 2018. I was responsible for developing and maintaining professional relationships with physicians of all disciplines, their staff members, as well as with staff at assisted living facilities, nursing homes, and skilled nursing facilities all while having the company's clinicians providing 5-Star care to patients in their homes. I had a monthly quota of home health referrals that I was expected to bring in per month and also be the professional go-between for the doctors and their patients. As a home health rep, I would make introductory calls to the patients once the referral was processed and informed of via email, explaining who I was and about my company, and what services were ordered to be provided, and what they were to expect from our clinicians from start of care to discharge from services. I would establish rapport with the patients and their involved family members and provide my contact information should they need to reach out to me with any problems, questions, or concerns with Activa Home Health and their care during the Medicare approved certification period. I did my best to make sure all goals and expectations were at least met, if not exceed, and ultimately was an extension of the ordering physicians and their practices. I was also able to take verbal orders from the physicians if needed because of being a current, licensed registered nurse in the state of Florida. I would also provide updates, progress, and status changes of the patients to the physicians on a regular basis and as needed. I cultivated and maintained my relationships with all my accounts on at least a weekly basis, if not more depending on the need. I also attended and participated in as many facility/community events as possible, whether it be to help out or observe in order to get my name and Activa's name out in the community.
