Responsible for the delivery of comprehensive nursing care to assigned
12 - 15 patient caseload in the home care setting. Provides assessment
of patient and family needs with the development, implementation and
evaluation of an appropriate Plan of Care. Ensures changes are made in
response to changing patient needs. Collaboration with the
interdisciplinary team, patients, families, physicians and caregivers .
Managed medications and provided education to caregivers to enable
them to provide end of life symptom management. Supervised and
provided guidance to LPNs and HHAs. Provided documentation in a
concise and timely manner into the electronic medical record. Mentored
new RN case managers as assigned by supervisor and took med and
nursing students out into the field to provide an introductory experience
with Hospice homecare.
Coordinated care with physicians and other clinical staff to prepare for treatment, carry out interventions and enhance continuum of care to deliver comprehensive services. Worked closely with the Pediatrician who founded the practice. Provided triage nursing, teaching to parents and referrals. Used newly implemented EMR system to update patient records, transmit prescriptions and transfer files. Administered medications, immunizations, treatments and provided health screenings.
Maintained and updated cumulative health records for all students in the building including immunization data;
Conducted scoliosis, hearing and vision screening and assists in physical examinations
Recorded attendance data and participates in an intervention program for recurring illness and other attendance problems;
Evaluated school‑related injuries
Provided first aid and emergency treatment of injuries and illness of students and faculty;
Advised parents of poor attendance, tardiness, and the need for additional medical attention;
Dispensed medication as ordered by physician and requested by parent in accordance with Connecticut State Law.
Orderd, inventoried and oversaw the storage of first aid and related health supplies and equipment;
Acted as a liaison between physician, faculty, student and parents when there is a health problem.
Provided skilled nursing to Special Education students ( trach care, Gtube feedings, ventilator monitoring)
Involved with team to provide healthcare to facilitate education in accordance with the law.
