An important piece to the process of leaving a job is to write a letter of resignation. Your physical therapist resignation letter can be simple and to the point. Tia Petersen,

Please accept this letter as my formal resignation from my position as physical therapist with New Parkland Physical Therapy Clinic. My last day of employment will be April 28th. It is necessary that I resign because my family will be moving out of state due to the fact that my husband has recently accepted a job offer.

Thank you so much for the opportunity to work with the patients and staff here at the clinic. I have thoroughly enjoyed the experience and do hope I am able to stay in touch with you and my co-workers. I have gained such valuable skills in my time here.

I will be sure to wrap up loose ends prior to my last day on the job. All of my clients will be informed and introduced to their new physical therapist, and my paperwork will be completed. Please Don’t hesitate to reach out to me at 000-000-0000 should you have any questions or require further assistance.

Best,

Shirley Sanderson
