- Conduct survey inspections and complaint investigations of behavioral health clinics providing mental health and substance abuse prevention and treatment services including county programs, private community clinics, hospital inpatient behavioral health units, and opioid treatment clinics located throughout Wisconsin. Surveys and investigations include reviews of the quality of care/treatment, privacy concerns, and confidentiality issues in inpatient and outpatient programs, day treatment, transitional residential programs, opioid treatment centers, community support programs, comprehensive community services, and crisis programs. Guidance, interpretation, and technical assistance is provided on applicable State codes or Federal Regulations.
- Conducted nursing home and assisted living facility complaint investigations independently on resident rights, abuse, neglect, mistreatment, and misappropriation of resident property ensuring resident rights and quality care and treatment was provided. Investigations included resident and family interviews, facilitating group interviews, facility staff interviews, medical record reviews, observations of resident care and treatment completed by facility staff. Determine deficiencies of State and Federal codes and document results of the investigation into statements of deficiency.
- Conducted annual recertification surveys of nursing homes and sub-acute hospital units to determine compliance with Federal regulations. Conducted biennial recertification surveys of assisted living facilities- community based residential facilities (including programs that provide mental health and substance abuse treatment), adult family homes, residential care apartment complexes- to determine quality compliance with State licensure requirements. Formulated results of survey inspections into health care deficiencies/citations where identified.
- Long term Peer Advisor who has trained staff members on complaint investigation protocols and survey processes, monitored workloads of new staff, conducted performance evaluations on new employees and gave feedback to Regional Directors.
- Active participant on systems improvement teams for: Behavioral Health- developed surveyor worksheets and tools for the new DHS 50- Youth Crisis Stabilization Facility codes, participated in DHS 75 re-write work group, developed policies/procedures for DHS 40 application processes, and participated in process updating of the Principles of Documentation guide. In the Bureau of Assisted Living- developed policies/procedures and trained staff for implementation of the Home and Community Based Waiver process which consisted of working with multiple state Bureaus and Divisions on a collaborative team.
- Completed Plan of Correction reviews and provided guidance on health care deficiencies issued related to State codes and Federal regulations. Referrals made to the Office of Caregiver Quality for credentialed and non-credentialed caregivers when required. Conducted onsite facility follow-up surveys/inspections to determine compliance with State and Federal health care requirements, to determine whether or not the Plan of Correction components were completed, and whether or not any imposed remedies and/or enforcement action issued by the Department was completed as directed.
