Experienced nurse care manager working with vulnerable populations in the community. Collaborated with current clinic leadership, to create a new program providing hospital follow-up calls to patients leading to a nearly 50% decreased in 30-day hospital readmissions and a significant increase in patients attending their hospital follow up visits. This project led to becoming certified in Care Coordination and Transition Management (CCTM), presenting a poster at the American Academy of Ambulatory Nursing (AAACN) followed by a publication in the AAACN's ViewPoint to disseminate impressive outcomes.
