This position was a "fill in" position for a nurse utilizing maternity leave.  My job responsibilities included managing the program to make sure it remained in compliance with all Medicaid regulations, assisting clients with the utilization of medicaid reimbursed services applicable to their needs, in home and phone assessments monthly, collaboration with area agencies to obtain in-home aids for clients, billing for services rendered, and in home assessments to determine acceptance or denial of new clients based on qualifications set forth by Medicaid for the CAP program. I remained in this position until the return of the nurse on maternity leave.

RN/Health Care Coordinator/Assistant Manager for an Adult Day Center. Responsible for delivery and coordination of healthcare services and providing direct patient care to the participants attending KOALA. Responsible for performing pre-admission health assessments for acceptance into program. Job responsibilities also included the following: problem identification, monthly physical assessments; care plan creation, implementation and reviews; medication administration, wound care, implementation of B&B training, VS and blood sugar monitoring, providing behavioral/emotional support and supervision for those with dementia and other disabilities, maintaining all documentation in medical charts;collaborating with family, physicians and other agencies to ensure continuity of care and utilization of needed services; providing staff, family and participant education monthly; assisting manager with United Way grant writing, supervision of staff; coordination of staff schedule to ensure staff/patient ratio is in compliance with State guidelines all while staying within the budget; leading the onboarding process for new staff, representing facility during monthly audit from local DSS, representing facility during quarterly audits from Veteran Affairs and the Health Department; assisting manager with participant eligibility documentation, and billing for the Child and Adult Care Food Program (CACFP); assisting Manager with the utilization and documentation of the Home and Community Block Grant funding, and assisting manager with Annual Adult Day Service Recertification. During the last three years of my employment, I also accepted the responsibility of assistant manager and took over the billing portion of services rendered at KOALA. I remained in this position until I was unexpectedly hospitalized at 28 weeks gestation with my third child. Delivery followed soon after and he remained in the NICU for six months thus eliminating my ability to return to work at the end of FMLA.
