For the past 13 months, I have been bedside in a level 3 trauma center ER. A fast-paced environment with stable and critically ill patients has helped enhance time management skills and prioritization of patient care. Prior to that, experience as an RN has been solely in a PICU of academic centers with roles ranging from bedside nursing to charging. Working in both a cardiac and medical pediatric ICU in Jackson, MS and Nashville, TN opportunities to learn about and assist in treating various illnesses have been appreciated. As a bedside nurse, assessment and nursing skills steadily improved in a fast-paced and stressful environment with fragile patients. As a charge nurse I was able to experience the importance of leadership and role modeling. Serving as a resource for newer nurses, a relief nurse for those who needed to step away from the bedside, and a communicator for the unit with the hospital supervisors and management. An APRN specialty will in pediatrics with a dual degree in acute and primary care is anticipated in August 2022.
Continuing 180 anticipated clinical hours with Batson Children’s Hospital and North Mississippi Medical Center, focusing on pediatric intensive care. Clinical hours are spent rounding with the healthcare team, formulating patient-specific plans, and following up with care with the patient, family, and team. With guidance from preceptors, advanced skills of lumbar puncture and intubating were conducted. Histories and physical exams were completed on various patients including septic neonates, those in DKA, asthmatics, and patients hospitalized with covid-19.
180 precepted clinical hours, Mississippi Medical Center with Batson Children’s Hospital and North Mississippi Medical Center focused on pediatric acute care. Clinical simulation lab time was spent inserting chest tubes, placing central and arterial lines, and intubating. One-on-one preceptor time was spent formulating notes for history and physical assessments. This semester, the focus of many patients was ruling out the emerging MIS-C for a variety of patients, one of which was diagnosed with atypical Kawasaki disease after an echo revealed dilated coronaries and a 12-year-old male in 3rd-degree heart block after a positive covid infection. Other patient experiences include pulmonary embolisms, ruling out intussusceptions, syncopal episodes, and patients with failure to thrive.
180 precepted clinical hours completed in Mississippi Medical Center with Batson Children’s Hospital and Children’s of Mississippi in Tupelo, emphasizing endocrinology, nephrology, and complex care. Type 1 and 2 diabetes mellitus patients, obesity, and thyroid disorders in an outpatient specialty clinic were seen with an endocrine nurse practitioner. Assessment findings, medications, and lifestyle modifications were discussed with the patients and families. A variety of nephrology patients were assessed and care was discussed in a specialty outpatient clinic with a UMMC nephrology doctor. These patients ranged from receiving kidney transplants due posterior urethral valves, to those with nephrotic syndrome, and multicystic dysplastic kidney. Patients followed by the UMMC complex care team included those with the palliative team. These medically complex patients receive services from various specialists and make the complex care team nurse practitioners or physicians their home medical team. The team works closely with nutritionists and social workers, ensuring the patients can be appropriately cared for outside of the hospital setting.
180 hours precepted clinical hours with North Mississippi Medical Center in a primary care clinic with care provided from birth to late adolescents. Patients ranged from well child visits throughout the lifespan to sick visits with a variety of viral or bacterial illnesses. One on one time was spent with my preceptors in performing head to toe assessments on both well and sick visits, formulating plans and diagnoses, and what follow up would need to be done.
