Perform nutrition assessment and intervention on required patients as per professional standards. Prioritize and provide timely and proper nutrition care for all patients. Assess, implement, and monitor patient's nutritional status and nutrition progress accurately. Monitor quality of food service as required per client. Assist with planning, implementing and evaluating nutritional care policies and educational programs as required per client. Maintain accurate records such as patient's nutrition care plan and nourishment/supplement records. Instruct patients and families on diet modification as necessary. Document nutrition intervention and patient's education on medical records in a timely manner as per client policy.
Complete comprehensive nutrition assessment of patient's nutritional needs. Diagnose, assess, evaluate, and monitor the nutritional status of individuals to prevent disease, treat illness, and promote better health. Instruct individuals and their families about food preparation and meal planning for various restricted diets. Recommend dietary modifications and provide counseling for weight management. Consult with physicians and other health care professionals to coordinate services to best meet the nutritional needs of individuals. Document, develop care plans, and report progress for individuals using the Nutrition Care Process. Inpatient screening, assessing nutrition status, and providing nutrition interventions on a daily basis, to infants, pediatrics, adults and geriatrics who have a wide variety of conditions that affect diet, eating and growth. Flexible to the shifting demands and nature of a pediatric team. Obtain timely and appropriate data and analyzes data based on evidence-based standards and nutrition practice guidelines. Monitor policies and procedures in area of expertise and communicate areas of concern that impact patient care, while establishing proper patient care procedures and act as a liaison with nursing areas. Participate in and identify continuous quality improvement activities in clinical areas to ensure optimal nutrition care outcomes are achieved. Maintain complete and accurate documentation of all appropriate information in the medical record according to the established policies and Federal and state regulations. Identify malnourished patients as well as patients at risk for malnutrition and work collaboratively with interdisciplinary team to identify appropriate interventions, resources or solutions. Provide nutritional care to a complex population of premature infants in the NICU and complete nutritional assessments, nutrition plan recommendations, follow-up and round with the NICU team.
