Currently helping a caseload of 13 female clients deal with substance abuse, mental illness and other concerns by delivering personalized counseling support. Conducting one hour individual counseling sessions weekly with each client. Completing two fifteen minute co-occurring disorder case managements and two recovery support case managements weekly for each client. Family counseling if client approves. Facilitate four groups weekly for all 31 clients. Focusing on domestic violence, anger management, processing and encounters. Provide crisis intervention when needed. Keep accurate documentation of all interactions with clients. Keep constant contact with clients referral agency. Such as, intensive supervised probation, probation, parole, pre-trial services, DCP&P and drug court. Responsible for continuously sending updates on clients progress in treatment. Complete monthly extension requests in NJSAMS and monthly treatment plans. Complete weekly billing. Submit requests for clients to attend medical and outside appointments. Monitor clients ten minute phone calls. Screening clients packages. Run rooms for contraband and non permitted items. Plan after care placement for each client. Complete the multi-disciplinary team case conference form and present it to the clinical team to receive approval for the proposed discharge plan. Discharge clients from NJSAMS and the agencies internal website once treatment is completed and provide clients with a completion letter.
Conducted clinical evaluations using ASAM criteria in NJSAMS. Utilize the knowledge of the DSM-5 and ASAM criteria for appropriate treatment placement. Formulation of treatment goals and objectives for successful treatment outcomes. Managed the flow of all new admissions. Designated tasks and duties to the team on a daily basis. Ensured every client is accounted for and completing all of the admission tasks in a timely manner. Updated the daily log once a task has been completed. Managed the phone system and completed follow up engagements for clients seeking substance abuse treatment. Conducted phone screens, assessed clients for appropriate level of care, and requested additional documentation as needed. Coordinated scheduling for clients with the director of admissions and scheduled transportation for clients with residential appointments or detox services. Case managed walk- in clients and individuals seeking alternative placement. Managed referrals that came in through the fax or email.
Other duties include: administered urine and property screens; created acceptance letters for clients seeking a change of custody; verified medicaid status and funding sources for treatment; updated the waiting list and calendar.
Shadowed supervising social worker throughout the housing units while meeting with assigned residents. Attended and shadowed all re-classification meetings. Planned and executed activities to engage with residents. Met with assigned residents weekly for one on one counseling sessions and entered data into the residents online chart after every session. Gathered documentation from staff such as teachers and correctional officers to create parole reports for residents.
Other duties include: Monitored residents while receiving incentive phone calls. Reviewed intake checklists with new residents; helped new residents complete their phone and visit lists.
