Responsible for providing case management services for 75 to 80 clients with developmental disabilities. Develop and monitor person-centered "Individual Program Plan". Complete regular reports, forms, and case notes. Maintain case records. Coordinate services with other agencies and service providers. Consult with clinical and other professional staff. Conduct home visits with clients, families or other care providers. Coordinate and attend interdisciplinary team meetings in a variety of settings: educational, supported employment, medical, mental health, housing. Prepare and present cases for agency team review and discussion as needed. Advocate on behalf of client.
