I was able to secured residence for a consumer within thirty days after he was admitted to the hospital following his mother's hospitalization. After leaving the hospital, his mother continued to require therapeutic services that could only be provided in a rehab center.Subsequently, she was moved into a nursing home setting. She resided in the nursing home for approximately two years, before she was able to move into a senior citizen apartment. shortly after, she moved her son out of residence and they are once again living together. Happy ending! considering that his mother was gravely ill and was not expected to live.
Develop, implement, the Service Plans for up to a 20-person caseload. Locate resources, programs, and other services for consumers. Assist consumers with maintaining eligibility for services. Coordinate monthly face-to-face meetings and home visits as needed. Assist in the process of investigation regarding possible abuse of consumers by interrogating the consumers and family members to ensure the future safety of consumers.
