Initially I worked as a provider of in home Nursing Care and was a case manager on a caseload of 15 to 20 patients including neonatal and Pediatric patients . I was responsible for performing initial and ongoing assessments and providing in home treatments such as wound care, IV infusion, Enteral feeding, Venipuncture, Tracheostomy care, Colostomy care, Foley and supra pubic catheter management, Medication management and education of pts and families in disease processes, medications and safety in the home setting. I worked with the team of Nurses, Therapists, Dieticians, Home Health Aides and Medical Social Workers. I was responsible for communicating with the pt's Physicians and family in order to obtain the best possible outcome for my patients while providing quality care.
I was promoted to the Position of Clinical Nurse Supervisor after approximately 8 years in the field. In this position I was responsible for overseeing a team of field nurses in a specified area which included 80- 120 patients. I provided support and assistance to the nurses as well as reviewed and evaluated there work and documentation on a routine basis to insure the pts were getting the quality of care required. I reviewed all lab work results and forwarded pertinent info to the Nurses and Physicians caring for the pts. I also fielded calls to the office from Patient, Patient's family, Physicians, DME Companies, Pharmacies, Referral Sources while maintaining adherence to confidentiality and P.H.I regulations.
In March of 2015 the Agency initiated an Intake team and asked me to take the position of Intake Nurse Coordinator. I was teamed up with a clerical assistant who was educated in the ICD-10Coding system. I was responsible for assisting with the intake of information from our referral sources and completing the necessary referral documentation. I reviewed all materials and info we received in order to provide the needed info and supplies to the RN's doing the initial evaluations in the field to make them as prepared as possible. I made welcome phone calls to our patients in order to explain what home care was and answer any questions they may have.
Unfortunately due to financial concerns the RN intake position was dissolved and in October of 2015 I was asked to step into the position of the Quality Assurance Nurse Coordinator. In this position it has been my responsibility to audit the quality of the care and documentation of the staff in performing their various tasks. I then conference with them and their team leader, to point out their strengths and weaknesses and provide them with information to assist them in correcting any issues. I also offered myself as a person resource to them to answer any questions they may have. I also am responsible for submission of patient survey data. I am also a member of the QA committee and Safety committee.
Throughout my employment with the agency I have been providing on call triage services for our patients, and back up on call for the field nurses on a regularly scheduled basis.
In my position as a Float RN I provided care to patients in various parts of the hospital including MED Surg units, Inpt SNF unit, Outpatient Surgery, ER Triage and ICU. I provided direct care to 8 to 15 patients depending on the shift. My responsibilities included Assessment of all body systems, Developing and implementing plan of care. I provided total patient care including assisting with personal care needs, administration of oral, SC, IM, IV, SL and topical medications. Provided wound care, managed nasogastric and G- tubes, Foley and suprapubic catheters, provided ostomy care, Monitored I& O, and performed Glucometer checks and pulse oximetry. I also have assisted physicians with bedside procedures. I also provided education to pts and their families. I always took pride in providing quality care while following all rules and regulations and I was always open to any educational opportunity to improve my knowledge and skills.
