03/2010 to 01/2012 Comprehensive Adolescent Pregnancy Prevention (CAPP) Program Specialist YWCA Of Rochester And Monroe County | City, STATE, Recruited 34 at-risk youths ages 13-18 into the CAPP Program at East High School to provide comprehensive, age-appropriate, evidence-based, and medically accurate sexuality education to promote abstinence, delay the onset of sexual activity and reduce the practice of risky sexual behaviors among youths. Provided 34 youths with information and access to comprehensive reproductive health care and family planning services for teens to prevent pregnancies, sexually transmitted diseases (STDs), and HIV/AIDS. Taught participants life skills and healthy alternatives to sexual activity to help them develop the skills needed to transition into a more positive and healthy adulthood. Facilitated weekly group sessions with 15-20 youths using the approved curriculums, Teen Outreach Program (TOPS), Be Proud, Be Responsible and Love Notes. Established and maintained working relationship and rapport with parents, grandparents, and legal guardians of teens and participants in the program. Also, maintained working relationship with principal, administrators, counselors, teachers, staff, health care personnel, and outside agencies involved with teens and participant. Attended monthly Student Support Meetings to discuss teens and participants' progress, grades, attendance, educational goals, challenges, and implemented plan of action to overcome those issues. Record Keeping - Responsible for completion and accurate resident's files, including progress notes and IPP forms on a weekly basis; completion of monthly systematic reports for the smooth operation of the Family Support Services program.
