As a LPN for 20 years I have had the ability to see health care change through the years. With this being said I have also been given the chance to performed in several different nursing positions. Duties were to oversee all MDS assessments that are required and submitted on a timely basis. Case manager of LTC facility. Provide management of residents in the facility. Ensure all new residents and family makes a smooth transition to new surroundings. Upon discharge out of facility. Obtain any orders or medical equipment required for discharge as needed. Other duties were to visit referral facilities, or physician to market our hospital and the services we provide. Medically assess any potential admits for appropriate criteria for admit.
