Provided ongoing intensive case management for Veterans (homeless, home visits, housing and food assistance, appointments, job opportunities, financial aid options, etc.) Identified opportunities for property owners to provide affordable housing for Veterans. Utilized a Housing First approach to work with Veterans to ensure placement and stabilization. Engaged Veterans in a critical time intervention strategy immediately during enrollment. Interviewed, counseled and identified Veterans by conducting psychosocial assessments to identify treatment and establish treatment goals. Developed and Implemented a successful substance abuse program called “Stable and Able”. This program is the only one of its kind currently. The "Stable and Able" device is unique in its ability to monitor and participate in a patient's treatment. Telehealth utilized to support monitoring "Stable and Able" patients.
Development and implementation of Advance Care Planning Group Visit (ACP-GV) within the facility and in Clinic Based Outpatient Clinics. Provides facility training, education and Veteran recruitment for the Advanced Care Planning Group Visit (i.e. Living Wills, Advanced Directives.) Travels extensively to all CBOCs to facilitate ACP-GV. Participates in vital national calls, trainings and interdisciplinary team meetings. Organizes and coordinates Advance Directives groups, providing educational materials, conducting training and charting in patient records. Establishes and maintains positive working relationships with employees, volunteers, consumers and stakeholders within the VA and outside community agencies. Networking within the community to build and strengthen healthy, lasting partnerships. Successfully conducted new hire interviews, effectively executed the complete hiring process from position creation to applicant selection. Trained employees on ACP-GV program, milestone and metric, OMAT and CPRS clinic reports. Advertised and promoted ACP-GV program, currently 33 percent of Veterans completed Advance Directives at SNHCS. Of the 20 facilities participating in the ACP-GV grant, SNHCS is the second highest ranked.
Promotes LGBTQ education and awareness to providers and patients within the VA and the surrounding community. Conducts LGBTQ trainings to staff and provides ongoing cultural competency guidance. Provides a SAFE Space to the LGBTQ Veteran population while assisting with connecting patients to necessary and appropriate personnel. Participates in national trainings and calls to improve the quality of care for LGBTQ patients within the VA. Gathers and manages data analysis associated with outreach in the community. Supports provider with Transgender Veterans Group. Liaison for the VA and local LGBTQ community center.
