Dear Ms. Fowler,
This letter is to formally announce that your position with Sheinlein Communications has been terminated. This is effective as of the date you were escorted out of the building, which was June 15th of this year.
As you know, you are being terminated, per company policy, for insubordination. This was in actuality your third strike in this regard. Each incident is a part of your employee file. We have used this documentation to review your performance and determine that termination was the only option. We even have a declaration, signed after your second offense, that you understood if there was not a notable improvement in behavior, termination could be the result.
Your final pay will be direct deposited to your checking account of record. You will receive any accrued or allotted vacation or personal time one week later. If you contact your Manager, you can walk her through the personal effects at your workstation that should be mailed to you. Human Resources will be sending you a package regarding COBRA eligibility, if you would like to continue your coverage. Otherwise, all benefits will be terminated 30 days after your termination date.
