Develop and oversee Registered Nurse delegation plan for each direct support worker performing delegated tasks as defined in the Medicaid Waiver Contract Medication management. - Applies to medications that are self-administered, assisted with, or administered during the time the participant is receiving paid waiver services.
Complete weekly medication assessment, dispensing, and storage.
Remove medications from home as appropriate.
Complete site supervision visits, document on supervision notes.
Attend Individual Service Planning (ISP) meetings and assist in the development of individual plans (IP).
Review/write quarterly reports documenting progress toward the outcomes of the IP.
Update medical protocols as needed. Update medication administration record as needed.
Ensure that there is a current physician's order in participant's chart for all medications that staff assist participation with.
Train all Direct Support Staff on medication assistance.
Report any adverse conditions/events to Program Director
Provides complete patient assessment, including vital signs and pre-examination procedures. Gathers and assesses objective and subjective data from the patient.
Documents data and assessments in the patient record.
May use a computer system and information technology to ensure standard documentation, coding, and completion.
Assists the providers in all aspects of the consultation, treatment, procedures, and follow-up care.
Executing medical regimens and nursing interventions as appropriate for the setting and their competency.
Patient health counseling and instruction, including meeting with families and caregivers.

Complete and document initial and routine evaluations, physical assessments and reviews as required by the programs administered by the AGENCY.
Participate, when appropriate, in the development of written and individualized program service and care plans.
Complete proper documentation for physician's orders.
Observes and reports to the Program Director or designated administrator and/or client's physician all unusual changes in condition, behavior or whereabouts that may require intervention. Monitors and supervises clients or activities as assigned.
Promotes and practices good hygiene and sanitation to all staff and clients by training each caregivers on Sanitation and infection control on their houses and also to the DP trainers in the Day program; requiring clients to wash their hands first before taking snacks and going to the lounge for their lunch and dinner.
Performs annual employee health screening in accordance with policies and procedures.
Participates and assists during emergency drills and situations such as fire, fire alarms, hurricanes, and other possible emergencies as outlined in the Disaster Manual.
Maintain client medication administration/HM's files/records.
Distribute new Med-Logs/Health Maintenance every 15th and end of the month to all caregivers.
Be responsible in calling in refills of medications to clients Primary Care Physicians and assist some caregivers in calling in for refills of medications at the Pharmacy.
Pick-up and records new and refill medications at the Pharmacy, count it and distributes to all caregivers.
Provide direct care and training services.
Transport clients for medical services or emergencies as necessary.
Ensures departmental compliance with corporate, state and federal safety guidelines.
Provides care in clients' homes island-wide. Wound care, dressing changes, vent care, G- Tube feeding, catheter insertion, injections, pain management. Homecare duties include but are not limited to personal care, companionship, running errands, assisting with the activities of daily living, such as bathing, dressing, grooming, transferring, range of motion exercises, assisting ambulation with wheelchair, walker, or cane, incontinence care, toileting, taking vital signs, meal preparation, daily paperwork/charting.
Responsible for providing nursing care through all phases of care taking into consideration age and developmental needs.
