  • The MDS Coordinator reports directly to the Director of Nursing and is responsible for assigning skilled ICD-10 diagnosis or long term care ICD-10 diagnosis for admission based on patients History and Physical and the patients Discharge instructions that are acceptable to the CMS for billing purposes.
  • Responsibilities also include completing Physician Certifications and MDS assessments for patients and submitting them to the CMS, and submitting patients PASSARS to the Oklahoma Healthcare Authority.
  • The MDS Coordinator is also responsible for preparing Care Plans for all patients based on information collected by working closely with a multidisciplinary team including nursing, social services, dietary, activities and physical therapy and meeting with families and patients monthly to discuss care and address any concerns they may have.
  • The MDS Coordinator is responsible for conducting daily PPS meetings with the business office manager, the social services worker and the director of therapy, including entering all therapy orders into the computer to ensure patients receive quality care and that all CMS guidelines are being followed.
