Heritage Manor South is in the VA Contract Nursing Home Program. As assistant director of nursing, I was responsible for all aspects of care for the veteran residents from admission to discharge. References for the following information are Joysaline A. Young, Contract Nursing Home Coordinator (resumesample@example.com) and Helena Dean-Reed, Contract Nursing Home Program Registered Nurse (resumesample@example.com). Below were my duties to our CNH Contract Veteran Residents:

  • Submit status changes regarding admission, hospitalization, discharge, or death with CNH Change of Status form.
  • Analyze MDS to determine RUG score for non-skilled and subsequent skilled RUGs for submission and approval with the OBVAMC.
  • Report the total number of residents tested for COVID-19, total number of residents with positive COVID-19 tests, total number of veterans with positive COVID-19 tests, total number of employees tested for COVID-19, and the total number of employees who tested positive for COVID-19 three times weekly for CNH/SVH.

  • Face-to-face physician appointments via VA Video Connect appointment for RUG changes related to therapy approval.

My other responsibilities and/or duties included:

  • Managed the high-risk weight management program for 80-90 residents in the 145-certified bed skilled nursing facility.
  • Weighed all residents once monthly to determine if the resident had a 5% gain or loss of weight within 30 days or a 10% loss or gain of weight in 180 days.
  • Collaborated with registered dieticians, health care providers, and nursing staff to determine and implement interventions specific to individual residents who were deemed high risk due to their weight loss or gain.
  • Tested all residents once weekly for COVID-19 as mandated per numbers of positive COVID-19 cases within Caddo Parish.
  • Conducted assessments of nursing staff performance and provided in-services and education for deficiencies or inadequacies.
  • Gave guidance to personnel as needed to evaluate performance and discuss any improvements needed.
  • Worked each shift on each hall as a floor nurse and a CNA to build relationships with the residents and nursing staff.
  • Worked and managed the COVID-19 unit as a floor nurse and a CNA while still successfully performing duties as the ADON.
  • Maintained accurate reporting and recording according to policy and procedure in order to evaluate and make recommendations for improvements.
  • Received report from charge nurses on each shift and worked closely with nurses to ensure maximum total patient care.
  • Participated in nursing care as needed and responsible for the direct handling of any emergency situations that arose.
