Primary Home Care RN Case Manager providing skilled professional nursing care to clients/families/caregivers as prescribed by the physician and in compliance with regulations as established by the State Board of Registered Nursing, the client, and the policies of Hospice Care of Southwest Michigan.
Duties included providing professional nursing care by utilizing all elements of the nursing process, assessing and evaluating the client’s status, initiating and sustaining, as needed, the implementation of orders for medications and treatments prescribed by the attending physician or designee, completing relevant clinical notes regarding the client's condition and care, individualizing and teaching the client/family/caregiver self-care techniques as appropriate, working in cooperation with the family/caregiver to meet the needs of the client/family/caregiver, observing the client for changes in condition and reporting these changes and the client's needs to the attending provider and appropriate members of the Hospice Interdisciplinary Group, assisting the client with activities of daily living and facilitating the client's efforts toward self-sufficiency and optimal comfort care.
Responsible for utilizing the nursing process to develop care plans and goals for an assigned case load of participants in order to provide coordination of care/services in partnership with participants, family members and the P.A.C.E. interdisciplinary team. Conducted in-home comprehensive assessments and care planning, provided chronic disease management and education for participants and families, coordinated care within Senior Care Partners, P.A.C.E., and other facilities. Coordinated all participant transitions. Provided compassionate care/support and collaborated with all those involved in participant care utilizing a participant-family centered approach.
