  • Facilitate daily interaction between patients, hospital case managers and physicians, coordinating discharge home from hospitals or skilled nursing facilities for those patients referred. Provide informationals to patients and their families to insure appropriate education regarding home care once discharged.
  • Serve as an effective hospital liaison with patients, physicians and case managers, assessing medical charts and promoting a high level of professional communication and interaction.
  • Established strong work network by building professional relationships with physicians, hospital case managers/socials workers and skilled nursing facility/rehab case managers/social workers for over ten years.
  • Organize special events for patients and families and provided educational resources to assist with emotional support. Facilitate a monthly elective surgery information class for patients and their families in regards to orthopedics.
  • Serve as an advisory resource by providing patient and family care expertise in form of educational materials, videos and the latest information in regards to home health and hospice.
  • Collaborate with multi-disciplinary staff to improve overall patient care and response times by our field staff.
  • Communicate with patients with compassion while keeping medical information private adhering to HIPPA guidelines.
  • Provide hospice informationals at the hospital bedside for patients along with family members present when deciding end of life care and wishes.
  • Assist our Intake Department in the handling of new referrals from hospitals, rehabs and MD practices for all Personal Touch offices in Virginia.
  • Order appropriate durable medical equipment for patients when needed as a convenience to insure a smooth transition home.
  • Assist in local networking events/provide community outreach to educate the public on the services offered by Personal Touch Home Health & Hospice.
Patient Services Manager, Leukemia & Lymphoma Society, October 2007-March 2009Hampton, VA
  • Assisted patients in finding local/national support services to include clinical trials. Assisted newly diagnosed patients with one on one support.
  • Planned/Managed six family support group meetings and provided back-up to all support group facilitators.
  • Assisted patients in applying for LLS's annual financial grant and provided education on LLS's copay grant program. Oversaw pre-award through closure.
  • Planned patient and healthcare professional educational programs with local hospital systems. Secured physician speakers and conducted community outreach presentations.
  • Assisted campaign staff with LLS's School & Youth Campaign, Light the Night Walk Campaign, Team in Training Campaign and Man & Woman of the Year Campaign.

  • Was responsible for direct oversight of PAF's first federal grant of 1.6 million awarded by the CDC to assist the underserved and underinsured.
  • Was a Director for PAF's Internet Patient Services Division that facilitated every email request for help nationwide.
  • Was a Regional Advocate Coordinator which led me to educate elected officials at both the state and federal levels about policy and legislative reform that would improve, restore and/or maintain access to care for patients nationwide.
