As one of Rocky Mountain Care's leading patient care coordinators I am responsible for the logistics and operations in coordinating patient placement from the University of Utah and Huntsman Cancer hospitals. Each placement requires specifically catered planning and scheduling within 4 separate departments of the company.   • Build and utilize reporting systems through technology management to keep customers and team members in the loop with latest information.
  • Simultaneously operate 4 separate healthcare specific technology systems, and manage an average of 250 e-mail corespondents a day.
  • Ensure confidentiality of highly sensitive data.
  • Provide the necessary support to other local public health agencies who are involved in patient care.
  • Process and place over 100 patient referrals each month. This process requires accurate, punctual, and highly organized collaboration to ensure customer satisfaction.
  • Regularly evaluate functions and strategically create protocols and guidelines to maintain a high standard of quality care systems.
  • Develop strategic relationships with general practitioners and coworkers to build a strong network of support to ensure accurate processes.
