  • 8) Cross reference between membership population and provider (doctor) population using addresses - address, cities, postal code, and state data. Comparing distribution of specialists to the member population that needs them i.e. Pregnant women to OBGYN, Cancer patients to Oncologists, members who have heart disease to Cardiologists, etc.
  • 9) looking at doctors panels and comparing to frequency of annual check-ups for members in panel, panels to periodic check-ups for diabetics. Verifying that doctors are seeing all patients in their panel in a timely manner.
  • 10) Comparing member discharges to discharge assessments. Looking at the best ways to avoid a re-admission. Ways include: A) Verify member is taking prescribed medication B) Verify member saw their PCP within 14 days of discharge C) Verify all required lab tests are done timely
  • 11) Look at member's with co-morbidities and compare lab data to pharmacy data to look for trends and predictive triggers are on stratification changes.
  • 12) Look at Psychiatric/ Behavioral Health conditions and lab and pharmacy data for members with 2 or more behavioral health conditions
  • 13) Look at members with Depression, Bi-polar disorder, Schizophrenia, Psychosis, and Dementia and review Rx and Lab data to compare to encounter data and compare to members with 2 or more behavioral health conditions
  • 14) Look at Health Profiles of members with Asthma, Diabetes, Hypertension and compare stratification to members that also have behavioral health conditions.
