I have been a Licensed Vocational Nurse for close to 10 years, I have experience as a Med-Surgical nurse. I have also experience as a home health nurse, pediatric private duty nurse as well as experience working in a skilled nursing facility. I have also worked as a clinical care coordinator in home health, managing the clinicians, and patient needs in my assigned area. As a charge nurse I am responsible for maintaining the physicians plan of care, direct patient care, assessing for changes in status, overseeing CNA's to ensure that all patient needs are met. Assisted patients with healing and recovery after surgery. Perform all tasks with a patient-centered focus while seeking opportunities for improvement of processes and treatments. Provide behavioral/emotional support and supervision for those with dementia and Alzheimer's. Assist patients with multiple chronic diagnoses including COPD, and asthma, as well as CHF. Evaluate patient care needs, prioritize treatment and maintain patient flow. Coordinate with physicians and registered nurses to develop care plans for patients. Test glucose and administer injections. maintain patient charts and confidential files. IV Therapy, set up and monitoring.
Direct patient care, assessment of patient current and changing needs, maintained direct relationship with physician, administer medications, perform lab draws, record new symptoms, follow up on new medications or treatments, report findings to physician, teach family and patient regarding medications, adverse reactions and disease processes. Familiarize patients and there families with support programs available in there communities.
