Facilitate workflow for evaluation of provider orders, progress notes, and prescriptions to Case Manager. Provides oversight in the Planning, Implementation, and Coordination of Care in the Community to ensure high quality cost effective care. Collaborate with administrators to share best practices and address common problems and complaints via quality assurance protocols. Ensure completeness of patient registrations, verify appointment scheduling, follow up on patient referrals. Organize and maintain patient charting process, prioritize and review charts for completeness. Work in conjunction with RN's on the team to ensure patient care has been rendered according to requested services in under 90 days of initiation of the care request. In conjunction with the RN, coordinate care for all basic care needs based on the CAN score of 74 or less and collaborate with the Registered Nurse and Social Worker on higher level coordination. Receive, review, and upload medical documentation to communicate care needs to Registered Nurse, ordering providers, and/or Care in the Community mid-level providers. Report all critical findings immediately to Registered Nurse and alert the ordering physician of the findings. Receipt of RFS's and route them appropriately to ensure continuation of care or requests are communicated to ordering providers. Collect daily metric analysis for review and report data to administrative staff . Assist with the hiring and interview process of new potential employees.
Provide primary Ophthalmic care to patients and act as clinical assistant to physician. Essential function include, but are not limited to performing refractory measurements for eye glass prescriptions, complete ophthalmic history and preliminary exams, sub-specialty tests (refraction, Visual field test, A-scan biometry (IOL master), color testing, fundus photography, and contact lens evaluation. Verifies patient information by interviewing patient, recording medical history, and confirming purpose of visit. Accurately records all information in CPRS according to established standards and physician requirements. Check conditions of patients' eyes by observing pupils, muscle, visual acuity, extraocular movements, and blood pressure (if requested by physician). Prepares patients for ophthalmology examination by dilating pupils, changes in visual acuity, elevated extraocular pressure or blood pressure and communicating results to Attending Physician. Assist the physician during diagnostic and minor surgical procedures sterile preparations and documentation. Administered topical medications or diagnostic drugs as required by the treatment. Provide patient education pertaining to treatment. Maintain confidentiality of sensitive information per HIPPA guidelines.
