- Receive, screen and complete assessment interviews with CCP (Community Custody Program) youth as well as their families - Facilitate the transition of the youth from the detention center to reunite them back into their community. - Assist CCP youth and their families in accessing, utilizing and understanding the juvenile justice process - Assist CCP youth in establishing and developing contact with support agencies - Development of a safety crisis plan with CCP youth and their families - Create and maintain service plans with social workers and/or counselors - Assist CCP youth in obtaining Social Service benefits including but not limited to Medicaid, WIC, Food stamps and housing - Complete and maintain proper documentation and records for CCP youth - Serve as a member of the Behavioral Health team and coordinate case management assessments with members of the Multi-disciplinary team - Network with outside agencies and other departments to ensure the proper care and processing of CCP youth - Provide supportive and nurturing guidance to CCP youth during case management process - Connect and maintain positive and professional relationships with community agencies, resources and professionals for the proper management and guidance of CCP youth cases -Maintain communication with Judges, Public Defenders, District Attorneys, and Juvenile Probation -Compose and send typed Court Reports to all parties involved with the CCP youth's case, which provides an update of the youth's progress -Receive and communicate Court Dockets -Attend Court on behalf of the program when needed -Assist in the Alternatives to Detention National Model Site Presentation to Juvenile Detention Sites Nationally -Present data and updates to the Juvenile Justice Board -Participate in the monthly Alternatives to Detention meetings that include (Juvenile probation officers, District Attorneys, Public Defenders, CYFD statistician, Community organizations, and Court Organized Programs) -Assist in ACA Audit in the YSC training department -Attend meetings to collaborate resources for our program/youth -Assist in creating and opening the South Valley Youth Reporting Center -Assist in creating new contracts for new organizations/programs -Assisted in scoring a proposal for the Reporting Center -Assist with programming at the Girls Reporting Center and Youth Reporting Center -Data entry/tracking on spreadsheets for quarterly reports -Data entry into MIS (Practima) Program for monthly statistical reports -Entering and distributing the outcome of the youth's Detention (Court) Hearing on word template -Entering Daily Census into MIS -Ordering of office supplies through SAP and through Sandia Office Supply -Filing court and office documents in the clerks office
The Children's Community Mental Health Clinic is located at the Youth Services Center and served youth involved in the Juvenile Justice System. I was responsible for the following duties: • Receptionist- Processing referrals (contacting families to schedule appointments, entering data into MIS, Checking Insurance Eligibility) * Maintain client data to assure all data was up to date and accurate. • Scheduling of all appointments • Checking clients in • Preparing client charts for the providers (Therapists, Psychologist, Psychiatrists) • Filing • Answering multi-line phones • Certified to place clients on Medicaid • Assisted with billing claims to insurance companies • Assisted providers by completing vital signs on client • Called clients for script pick up • Typed client progress/court documents/letters for the Therapists, Psychologist, Psychiatrists
