Assessment of clients for in-home care, conducting visits for wound care, including wound vacs and other specialty materials available though wound representatives, medication set up, in-home medication teaching, coordination of care with other health disciplines. Assisting the owners and Director of Health Services for Manatee with credentialing for private insurance and Medicaid supplement plans. Marketing for the company in Sarasota and Manatee to doctors, assisted living and skilled nursing rehabilitation facilities for the in-home services to be provided post DC from facilities.
Assessments of patients with various medical diagnosis ranging from post-trauma, spinal cord injury, stroke and orthopedic surgeries, medications administration, coordination of care for patients in an inpatient facility for rehabilitation. Working with therapy team, case managers and physicians to optimize the patients ability to return home. patients are still able to receive acute care needs in a rehabilitation setting. Nurses are responsible for IV blood draws, IV insertions, and care of central lines.
Development of work flow for a two physician office that was acquired with expired supplies, equipment in non-working order and frequent turnover. Organized supplies to meet basic clinic standards, and removed expired supplies from inventory. Inventory taken of vaccinations, which could be later be entered into the EMR for inventory and charging purposes, trained five medical assistants/licensed practical nurses in a five week period. Training of staff included use of the EMR, proper documentation of injections, use of the EKG and spirometry machine, as well as other procedures that arose in the family practice setting. Facilitated getting one physician from using paper forms during visit to entering diagnosis codes, labs, imaging orders, referrals and other EMR information into tablet during the patients visit. Due to frequent turnover of front end staff and back end staff, was unable to establish a set work flow that would have established retention of staff. Developed written policies for glucometer use, urine reagent test strips, influenza in-house testing and strep in-house testing to help meet accreditation standards. Developed a training manual/orientation guidelines to incorporate policies, procedures and training manuals provided by EMR company.
Supervision of RN students in hospital clinical setting, with medication pass, instruction and hands on guidance of clinical skills need to be an RN, including assessments, facilitation in critical thinking, and collaboration with unit nurses in the hospital to assure that the RN students are able to maximize the learning environment during the clinical day. Coordination with various clinical units in the hospital setting for observational experiences.
Admission of clients in a home and rehab settings to ensure that clients are safe and able to return home with nursing and/or aide assistance, with care requested by the client, ensure the clients will be able to function safely within the home setting, supervisory visits every 60 days with clients to reassess needs, supervision of caregivers and field staff nursing, handling of calls related to customer service, new physician orders, scheduling, and caregiver concerns. Annual, bi-annual and complaint surveying of health care facilities, to ensure that the facility is in compliance with Federal and/or State regulations. Duties included writing of deficiencies when in violation to be posted on the State of Florida regulation sites, and ensuring that new facilities were in compliance with standards prior to opening with residents/patients. Facilities surveyed included assisted living facilities and nursing homes. Investigation of complaints for facilities regulated by Florida licensing, which were filed by the public to ensure that facilities are in compliance with regulations. When not in compliance, deficiencies were written and coordination with other regulatory agencies were filed as need.
One of four RN’s that assisted in the implementation of concurrent core measure review nurse positions at SMH in 2009. Worked with floor nurses, manager and physicians in the education of core measure criteria for heart failure, myocardial infarction, pneumonia and surgical inpatients, abstraction for Medicare/Medicaid reimbursement, collaborating to include criteria in order sets and protocols for nursing and physicians.
