As a Care Manager I primarily work on linking my clients who have, but are not defined by, Intellectual and Developmental Disabilities, with supports and resources to increase or maintain their independence.


  • Facilitate and lead Life Plan meetings with the individuals I serve, their families/guardian or advocates and with their providers if they are participating in any programs.
  • Coordinated referrals to specialists, hospitalizations, ER visits, ancillary testing and other enabling services for individuals.

  • Provide advocacy for and with members so that they feel they have a voice and are working towards the goals they feel are important to them.
  • Maintained an organizational system to ensure that all regulatory trainings and individual assessments, and Life Plans are being completed to ensure compliance.
