Identify the needs, wishes, and desired outcomes of each individual and/or their family. Develop an Individual Support Plan (ISP) in collaboration with the individual and/or their family, and those identified as part of their team. Locate the supports and services identified in the ISP. Identify skills and link to employment opportunities. Work closely with Office of Vocational Rehabilitation (OVR) to obtain and enhance employment opportunities. Connect the individual and/or their family with service providers and community resources. Coordinate the identified supports and services as request by the individual and/or their family. Monitor the supports and services identified in the ISP and assure the health and safety of the individual. Assist individuals and/or their family in apply for medical insurance, identification, and eligible benefits. Assist individuals and/or their family in applying for medical insurance and benefits for which they are eligible. Assess for needed supports and services throughout service with client.
Work with court adjudicated and potentially assaultive youth requiring treatment and specialized program services in a secure/restricted facility/setting. Implement treatment plans. Supervise daily activities. Report progress in attaining program goals. Respond to emergencies and use physical restraint when necessary. Assist juvenile youth in the job application process (e.g. filling out correctly, identifying skills). Instruct youth in life skills, interpersonal skills, communication skills, and decision making skills.
Work includes assigning and directing staff to escort or accompany adjudicated youth during their daily activities; supervising the use of restrictive procedures for youth that may be a danger to themselves or others; overseeing staff on special assignments or visits; assigning staff to search for escapees or adjudicated youth who are absent without leave and to aid in their return when appropriate; and providing or supervising instruction of adjudicated youth in areas including, but not limited to, life skills, interpersonal skills, communication skills, and decision making skills. Work also includes inspecting living and common areas to ensure that materials, supplies and equipment are adequate to meet the needs of the adjudicated youth; making frequent rounds of assigned areas to ensure the safety and security of the facility; and providing guidance to and inspecting the work of subordinates. Work is performed under the general direction of an administrative and is reviewed through observation of the behavior of the adjudicated youth and conferences and reports. Provide security by maintaining constant vigilance through the supervision of residents and employees in order to provide a safe living environment, which includes the prevention of harmful behaviors by residents towards self or others. Ensure the safety and security of employees, residents and the community by preventing residents from absconding and training employees on daily activity policies and procedures. Supervises Mental Health Techs who assist professional staff in implementing youth treatment plans. Train, develop and assist subordinate employees in the agency's principles and practices such as effective care and custody and intervention with overactive, emotionally disturbed and/or suicidal youth in order to improve the quality of care and programming for residents, through the following: Conduct regularly scheduled supervisory conferences. Observe subordinates during the performance of their job duties. Review subordinates' performance with them both orally and in writing. Complete the Employee Performance Review process for all subordinates according to MHT policy. Assign and direct employees in facilitating the scheduled recreational and therapeutic activities in order to provide residents with consistency of care. Ensures the timely and accurate completion of subordinates' reports, logs and forms to maintain accurate records of daily observations of youths' progress in meeting the program and individual goals. Receive grievances and complaints, conduct initial investigation into causes and conditions, discuss with employee or resident, and resolve any complaints or recommend solutions. Report employee infractions, follow-up on disciplinary problems and incident investigations, conduct pre-disciplinary conferences, recommend appropriate action and administer any discipline, as directed by the Human Resources Office. Document, complete and/or approve/disapprove employee-related paperwork including but not limited to requests for leave and overtime, work-related injury reports and required policy sign-off sheets in accordance with policy. Manage and control overtime by scheduling employees in a manner that effectively utilizes the workforce by moving employees to fill shift vacancies and scheduling employees in order to maintain a fiscally sound program.
