  • Co-created eForm application for the Family 48 Immunization Clinic on October 6, 2012. The Family Flu Clinic provided immunizations to approximately 4,500 Mayo employees and family members within a seven hour period. The application developed for the event featured a preregistration system, patient tracking system, a vaccination ticketing system, and a system to help the fl ow of patients and their information throughout the clinic.
