Maintain caseload We are responsible for doing chart audits periodically throughout the year to verify the accuracy, completeness of the chart/records, and establish that it meets required compliance standards. I am personally responsible for. documenting my individuals appointments on a doc sheet as well as all compliance required documentation in their charts.I file contact records for all phone calls made with individuals. I review my charts periodically to ensure they are meeting all State and County regulations. I am required to do daily documentation sheets on the individuals i serve in regards to the services i provided to the individual during our appointment. I include discussion of their individual goals, any information provided to them during our appointment, as well as any decisions made about their treatment or recovery. I proofread the doc sheet myself prior to submission. It is my sole responsibility to make sure these doc sheets are completed entirely, accurately, and submitted on time to maintain compliance. I have to personally interpret policy guidelines for eligibility and determine if action is needed if individual is not maintaining compliance. I have to personally interpret policy guidelines and determine if an individuals home is safe for me to enter to provide services. All of this has to be documented and submitted.
My job consisted of working with children between the ages of three and 14 who had behavioral health problems such as ADHD, ODD, etc. I had CPR training as well as Handle with Care training for this job and I was certified. I was also required to keep documentation of every child's daily activities and behaviors in a formal document. I have very good people skills and had to think quick on my feet due to the consistent change in children's behaviors daily.
