Merged a risk program and patient safety program to one effective and efficient risk/patient safety program at Trumbull Memorial Hospital. Completed two premium credit reports and both reports exceeded corporate average. Successful completion of risk assessment and action plans for each department in the facility. Developed and implemented successful action plans: action plan for falls resulted in an 83% reduction in injury from falls from prior year, action plan for pressure ulcers successfully remains below bench mark for hospital acquired pressure ulcers, hand-off communication action plan increasing awareness, reporting and emergency department had a 50% improvement in completion of SBAR forms. Worked closely with Chief Quality Officer (CQO), medical records, physicians and staff in reduction of all hospital acquired conditions from 17 in 2011 to 5 in 2012. Developed an effective Infection Control Program at Trumbull Memorial Hospital. Worked directly with managers and staff to greatly improve hospital acquired infections throughout the facility. Revised the entire Infection Control Policy Manual. Worked closely with previous Risk Manager on several Risk Assessments and Action Plans. Promotes best practices that involve infection control and risk/patient safety. Spearheaded the Flu Vaccination Program "Flu Fighter", Hand Hygiene "Black Light Challenge", Scrub the Hub, I Believe in Zero CLABSI, Zero VAP and Cleaning of Equipment Between Patients. Implemented a system to improve communication of isolation patients on the patients face sheet. Completed several initiatives involving C Diff, MRSA, and CLABSI.
