My name is Jasmine Unique Laguna, and as a kid I was never really fond of my middle name being an adjective but as I grew into the young woman I am today, "Unique," is the best way to describe how I can be at times. I am currently 22 years old and was born in January of 1999. I have a wide variety of diverse interests and love enjoying the smaller things in Iife. However, I am also a seasoned medical records clerk and I have obtained close to 2 years of experience and knowledge of being apart of both the medical record retrieval field and the medical record request processing/filing/scanning elements as well. I offer such an amazing energy to the office and in turn I hope the position I apply for can offer me opportunity for growth. I contain quick wits, strong mathematical, and intellectual skills that can provide any office or work place a very great addition to the team! I have been in the workforce since I was 18 years old and have racked up a variety of work experience and knowledge.
With the food handling, retail/customer service, warehouse and legal document retrieval skills I contain, I can obtain ANY skill, and ANY job I set forth my mind to. I just need the opportunity, thank you!
My position at LLUMC was a seasonal opening with the medical staffing agency I went through, just for 3 short months. For this seasonal position I would handle all Loma Linda UMC's outpatient and in-patient medical records, and our sister hospital's and outpatient clinics. I would sort records by date/time/patient/hospital/record type on a daily basis. The hospital's medical records would be dropped off by a physician to my off-site clinic in order to be prepared by me. It is very important I had the documents scanned and indexed into our company software for the patients physician to view their patients records ASAP in order to properly provide treatment. My department was backlogged with unscanned medical records for 3 months due to unprecedented events. However, with my assistance, 3 months worth of patient records were documented and photo copied into patient charts in a timely manner. Typing at least 45 WPM is a skill I contain as well. Being a document imaging specialist requires great attention to detail and leaves little room for mistake. This job required I have advance computer skill, use of office scanner/copy machine and many other equipment needed to fulfill the job, and a good work ethic in order to perform and I managed this position to the best of my ability. There are audits performed by mangers/supervisors and I have always met my supervisor's expectations. Other job duties include:
This position is where I gained most of my knowledge and skill to be involved in the medical/clerical field. My typing skills, accuracy, speed and efficient medical record processing came from my willing to learn and my willingness to accept constructive criticism to advance my knowledge. Compex legal services is a 3rd party record retrieval company who work on behalf of lawyers, attorneys, and insurance companies to collect medical/billing/radiology/or employment records from hospitals, employers, and clinical offices to be collected and provided to our clients for litigation purposes. Compex Legal services acts as a call center with workers being assigned states to call and submit our record requests to depending on our client needs. I would have to speak to record custodian's at multiple different hospitals/offices and developing relationships with medical record custodian's is what I excelled in. Establishing communication with the person fulfilling our client's record request is important in completing requests much faster. In order to perform and follow the productivity requirements for my position I would have to ensure I have speedy typing skills and able to speak to any person over the phone with confidence and to the point. Some other job duties are issuing payment for patient records if need be as record releases require patient authorization and payment for the amount of pages the records turn out to be. If records were ever released to me via email or fax, I would store the medical records/bills or films in our company data base for processing so they may be delivered to our client for their trial date as soon as possible.
I would fax, email or mail medical record requests that were HIPAA compliant to the hospitals/clinics that I was assigned to. I was also in charge of calling and following up with the record custodian's weekly on the status of the medical record request and document the status of record requests in our company software for our client to view and escalate orders if necessary. Documenting status of requests and ensuring I escalate my calls to office managers to avoid non-compliance from the location was vital.
A few of my job duties included:
My position as an Exceptions/Auditor for this company was very vital. I was in charge of correcting and updating stock counts and inventory as it was received and shipped. Inventory management is an enormous part of working in a retail warehouse where inventory is constantly being received, packaged and shipped. The exact count of inventory is to be recorded and reported to my supervisor accurately.
Mathematical skills, Writing/reading skills, Typing skills and computer application (Microsoft Excel, Word, Outlook) skills were also used on a daily basis in order to perform the needed tasks of an exceptions clerk. My job title was also responsible for auditing. I had to ensure packages that were caught in our sorting machine and transferred to my auditing station were correctly assessed and determined to be shipped to the correct mailing address and placed into the correct loading truck for its destination. It was very important that the weight of the packages are accurate and all items ordered off of the HSN website were included and not missing from our customer packages.
I would record and log inventory cycle counts into our company data application daily and record each audit that was passed or failed by the employee who had picked the package and log their performance for their supervisor to view. A few additional job duties I performed were:
As a retail store associate, customer service is key. This position provided an abundant amount of knowledge and experience regarding customer relations and I enjoy working and speaking with customers to get a better understanding of what is expected of us employees when our customers arrive at Rainbow Clothing. A few of my job duties included stocking the floor with merchandise we receive from our vendors. Engaging with customers upon entrance. Perform till counts periodically to ensure accuracy. I would organize the floor meticulously to allow our customer to find specific items and specific sizes that meet their needs. Rainbow Clothing also allowed me to input ideas where I thought something else would be best and became useful to the store in many ways. Additional duties include:
I was attending high school at the time I was hired at this job site and I did an amazing job balancing my school and work life. This was a seasonal position AKA a summer job that I applied for and my performance as a Cabana Waitress was always noticed. I was in charge of a handful of cabana's daily that would include 5-6 people and attending to their needs. I would provide food service, cleaning service and security to each cabana that was assigned to me and my customers were pleased with my attentiveness. I was a part time employee but would always volunteer for overtime if need be to clean up our work stations and soda machines etc. A few additional job duties include:
