I prepared and implemented individual service plans, quarterly reports, monthly updates, behavioral support plans, independent living skills assessments, maintained medication logs and discharge summaries for the children on my caseload. I assisted the children in successfully completing the goals set up in their respective service plans, which included making referrals for services necessary to complete those goals. I also worked closely with and maintained regular contact with the parents/legal guardians, therapists, attorneys, etc. for the duration of the children's stay at the Evans Home. I maintained ongoing case documentation and files for each child. I scheduled all the residents' medical, dental and therapeutic appointments, and maintained the master calendar for the Evans Home all year round. I transported to and from, and sometimes attended as necessary, appointments with the children. I attended school meetings, as needed, for Individual Educational Plans or disciplinary actions. I attended and participated in court and/or treatment team meetings, which involved DSS, attorneys, individual therapists, family members, probation officers, mentors, etc. and worked collaboratively with all professionals involved in the life of each child. I attended and participated in weekly staff meetings. I was in regular, daily contact with each resident and counseled them on day-to-day issues, but also when they were in crisis; this could include having to take them to the hospital emergency room if they were a danger to themselves or others. I filled in as a house parent, which included staying overnight at the Evans Home, administering medications, as necessary, and providing daily care for the children who live there. I communicated with referring agents regarding possible admissions and followed up with referring agents on acceptance or denial of admission. I attended CSA Symposiums/Vendor Fairs to share information regarding the Evans Home with referring agents throughout the State of Virginia. I provided ongoing support to the residents of the Evans Home alumni house and other former Evans Home Alumni, sometimes helping them with tax returns, advising them on college and proofreading their papers, employment or relationship issues, being a reference on job applications and assisting them in obtaining social services, mental health, medical and dental services in the community, etc.
I was responsible for 35 children in foster care, ranging in age from newborn to 18, possibly older if they graduated at a later age. I facilitated, and sometimes supervised, visits between the children and their biological families. I assisted parents in finding appropriate services to facilitate the successful return of their children to their care. I prepared service plans and attended court hearings on behalf of the children in custody. I placed, and transported, the children in foster homes or residential facilities throughout the state of Virginia based on their level of need. I set up appropriate services for the child, such as therapy, parenting classes, psychiatric medication management, tutoring or special extra-curricular activities. I maintained regular contact with the children and their parents by phone, letters and face-to-face contact. I attended necessary school meetings for Individual Educational Plans or disciplinary actions. I attended treatment team meetings for the child and work collaboratively with all professionals involved with the children.
