My name is Jessica Claire and I am an Administrator/Residential Monitor, subcontracting to the agency, Advocate Community Resources.

Advocate Community Resources (ACR) is an agency that provides residential support to individuals with developmental and physical disabilities within the Calgary and Southern Alberta regions.

ACR is funded by the Alberta Government's Persons with Developmental Disabilities (PDD) sector. ACR receive individual referrals from PDD, determine if the referral is the right fit for the services we provide at ACR, and, if so, find those individuals a 'Support Home' placement.

A 'Support Home,' is a regular home, where the home owner/renter becomes the 'Service Provider' to the individual with disability. Service Providers are subcontractors and they sign an annual Service Agreement with ACR to provide Service to a disabled individual according to ACR's policies and procedures. Service Providers are vetted based on submitted resume materials which outline experience working with people with disabilities, ability to adhere to policies and procedures, training and compassionate and caring qualities.

My role as an Administrator/Residential Monitor is to oversee a caseload of up to twelve Support Homes and Service Providers. This involves a variety of roles and skills that are outlined in the 'Experience' and 'Education' section of this resume.

  • Assist and collaborate in advance care planning for individuals receiving service as they age
  • Document and file Individuals health reports, consent forms, Individual Service Plans, Quarterly Summaries and Service Provider training.
  • Develop annual Individual Service Plans for Individuals receiving service, collaborating and consulting with various stakeholders including, private and public guardians, government representatives, community supports, family etc.
  • Collaborate with treating Psychiatrist, Healthcare Professionals, Practice Review Committee, Individuals, Community Supports and Service Providers to develop Behaviour Support Plans for individuals with behaviours of concern.
