Provides morning care which may include bed bath, shower, whirlpool, oral hygiene, combing hair, back care, dressing patients, changing bed linen, cleaning overbed table and bedside stand, straightening room and other general care as necessary throughout the shift. 3. 1. Provides general restorative nursing care and delegate formalized therapy tasks, as assigned, completing work accurately, safely and in a timely manner. Specific care to include: passive and active ROM, ambulation, special positioning techniques, splints, assistive feeding devices/adaptive equipment, ADL training, bowel and bladder management, restorative dining, bladder retaining. 2. Receives specific instructions from formalized therapies when the functions are beyond basic restorative nursing scope and receives supervision for these tasks from the indicated formalized therapy discipline. 3. Receives specific instructions from nursing on restorative nursing techniques through nursing orders. 4. Works with other disciplines, as appropriate, in setting up grooming, exercise, sensory stimulation classes etc. 5. Participates in restorative dining program and works with OT and other nursing staff. 6. Coaches and assists regular CNAs in positioning, ROM, ambulation, ADL, cones/splints/contracture care, ADL specifics, safety device application, appropriate padding and wheelchair cushions, safety device release. 7. Follows protocols and procedures for restorative nursing care as set by nursing administration. 8. Attends care planning conference and provides input into development of individualized plan of restorative care. 9. Informs charge nurse of suggestions for changes that she/he perceive as necessary for improving resident care and that would help a resident to reach their highest possible functioning level. 10. Reports lack of functioning equipment and lack of adequate supplies to supervisor. 11. Assists with bladder retraining and works with staff in seeing that the program is properly carried out. 12. Documents progress of residents by completing flow sheets daily to signify that the specified restorative nursing measure was done. Makes a weekly observation, progression/regression note in narrative formal. 13. Notifies charge nurse and DON when a person is regressing/progressing or refusing to participate in the program so that an assessment can be made by the nurses and the care of plan may be changed. 14. Assists with orientation and staff development in principles and practices of restorative nursing as directed. 15. Works with CNAs, as necessary to provide one-to one when a resident transferred from Level ll to Level lll. 16. Lets charge nurse and supervisor know when aware restorative nursing measures are not being consistently done in Level ll and resident is regressing. 17. Assists charge nurse in keeping lists of restorative approaches current and updated. If these measures are not on care plan, informs charge nurse and supervisor so this can be done. 18. Informs charge nurse and supervisor if an approach is on the care plan that is unrealistic and that you know is not happening so that this information can be changed so that the care plan accurately reflects the resident's current status. 19. Understands the three levels of rehabilitation and the two phases of a restorative nursing program and assists with interpreting this to the staff. 20. Services as a resource and teammate to other nursing assistants, working with them to provide the specific restorative nursing care for each resident. 21. Meets weekly with DON, PT Manager, MDS Coordinator, and Admission Coordinator to review restorative program and report resident progress. 22. Treats residents and their families with respect and dignity. 23. Provides reality orientation in daily care. 24. Prepares residents in restorative feeding program for meals; serves and removes food trays and assists with meals or leads residents, if necessary. 25. Distributes drinking water and other nourishments to residents with snack cart and hydration cart. 26. Answers residents' call lights, anticipates residents' needs and makes rounds to assigned residents. 27. Assists residents with handling and care of clothing and other personal property (including dentures, glasses, contact lenses, hearing aids and prosthetic devices). 28. Transports residents to and from various departments, as requested. 29. Reports and, when appropriate, records any changes observed in condition or behavior of residents and unusual incidents. 30. Participates in and contributes to interdisciplinary care conference. 31. Must be able to follow directions, both oral and written, and work cooperatively with other staff members. 32.
