Summary

Well-qualified transitional care coordinator adept at setting up services, working with healthcare professionals and families and giving patients continuous support through counseling. Good communicator and relationship-builder with exceptional skills in planning, resolving conflicts and solving diverse problems. Licensed RN in Ohio.

I’m interested in a position that will challenge and broaden my skill set as a registered nurse. Currently as a care coordinator, I’m learning a lot about the other side of nursing such as discharge planning and working with patients and the care team to ensure patients have successful discharges and are able to obtain the services they need to prevent re hospitalizations. This is an incredibly important part of healthcare that I would like to have a larger role in over the course of my career and wish to build my skillset in a new setting with new challenges.
