  • Completes family assessments, forensic investigations and case management for families with substantiated reports of suspected child neglect and abuse as defined in the General Statutes of North Carolina.

In-Home Services Senior Social Worker07/2017 to 11/2017
Forsyth County Department of Social Services – Winston-Salem, NC
  • Assure that abused and neglected children in their own homes receive a minimum sufficient level of safety and care and that they are removed from situations where that is not happening.
  • Case Management: Locates and provides resources for each family member, including the child (ren) to assist them in meeting their identified individual needs.
  • Assist the family and child (ren) with learning to reduce the excessiveness; severe and intense situations that have caused abuse and neglect.
  • Advocate for the family and child (ren).
  • Continuous assessment of the children's safety and risk levels throughout the life of the case and make CPS reports if it at any time it is determined the children's safety is currently being jeopardized.
  • Communicate and work closely with the Child Protective Services unit in order to obtain or provide information concerning new reports received on the families when In-Home Services is already involved.
  • Staff cases weekly with the Child Protective Services Social Worker and Child Protective Services Social Worker Supervisor when cases are assigned to In-Home Services due to the findings being Services Needed.
  • Completes ongoing assessment of the family and children's environment.
  • Communicate on a weekly or bi-weekly basis with professional collateral contacts throughout the life of the case to obtain information on the family's progress or lack of progress.
  • Complete weekly or bi-weekly face-to-face visits with the family and the child(ren).
  • Assist the family in developing their In-Home Service Agreement to address their specific identified needs.
  • Review the In-Home Service Agreement with the family during each face-to-face visit.
  • Complete 30 and 90 day Child and Family Team meetings with the family and their natural supports to determine what will or is working for the family.
  • Thoroughly updates Family Risk Reassessments and Family Assessment of Strength and Needs as needed as well as according to the North Carolina Division of Social Services policy.
  • Acts as a change agent, utilizing systems of care approach; which is based on objective diagnostic study of client(s) in interaction with his/her environment.
