Responsible for the effective supervision of offenders placed on community supervision. Conduct case supervision in a manner consistent with TDCJ-CJAD (Texas Department of Criminal Justice-Community Justice Assistance Division) standards and as envisioned by the Director. Responsible for compliance with the Mental Health Initiative (MHI) Special Grant Conditions as well as Domestic Violence Caseload Special Grant Conditions. Knowledge and experience in working with Local Mental Health Authority and TDCJ-TCOOMMI Community Based Interventions and Support Services.Maintain a caseload of misdemeanor /felony cases adhering to all CJAD, Department, and Court requirements; Responsible for monitoring the offender' s compliance with the orders of the Court through routine office and field contacts to ensure community safety; Must adhere to Special Grant Conditions when applicable in Special Programs. Responsible for assisting the offender in locating services needed to bring about pro-social , law abiding behavior;
Responsible for conducting assessments and individualized case supervision plans; Responsible for conducting orientation /intake with new probationers; Responsible for reporting violations of probation to the appropriate authorities in a timely manner ; Responsible for transporting offenders in accordance with Department policy and procedures or pursuant to lawful court order;
Develops and maintains case records by assuring each record contains chronological recordings of all significant actions, decisions, services rendered, assessments, re-assessments, Pre-sentence investigations , periodic evaluations and written criminal history; Maintains confidentiality; treats PSI (pre-sentence investigation) and all information obtained in connection with the PSI reports as confidential and releases this information only to persons authorized by law and directed by the court for the effective supervision of the offender; insures medical and psychiatric records obtained by the court are sealed or kept separate from offenders case files; releases such information only by order of the court; Able to communicate effectively with hostile , abusive or irrational individuals; establish and maintain effective working relationships with co-workers, County employees and the general public; Contributes to team effort by accomplishing other related duties as needed.
As a Qualified Mental Health Professional (QMHP), provides on/off site face-to-face emergency screening assessments and rapid crisis intervention/management. If inpatient stabilization is needed, provide on-going coordination with a psychiatric facility/hospital for patients experiencing a behavioral/emotional crisis with the potential for life threatening behavior and/or acute psychiatric crisis. Provides patient tailored community referrals and linkage to assist the patient in meeting individual needs and psycho-social requirements to include psychiatric needs. The least restrictive environment will be used to stabilize the individual. Is responsible for intra-agency and inter-agency coordination and referrals. Ensures that continuity of care and proper clinical follow-up is conducted if patient is not referred for inpatient treatment. Documents any service delivery, intervention conducted, communication with others to include outside agency personnel with rationale for intervention, date, time, name of person spoken to and purpose. Reviews clinical documentation for quality, completeness, and timeliness. Provides on-off site face-to-face screening assessments to patients requesting psychiatric outpatient services. Determination of eligibility is established, provides proper and clinical referral and intervention based on outcome of outpatient service eligibility. Provides coordination to community resources and services as clinically indicated. Adheres to Center Safety Policy and Procedures regarding training requirements and credentialing in related position. Acquires and maintains all credentialing requirements as QMHP. Submits accurate MAC time study data to include bubble sheets in a timely fashion. Adheres to Center Safety Policy and Procedures regarding training requirements and credentialing in related position
Conduct initial intake with client and family/guardian.
Ensure the timely completion of all Intake paperwork, Assessments and Individual Service Plans in accordance with Southwest Key Programs (SWK) policy.
Ensure all daily documentation is completed and entered into SWK database in a timely and efficient manner.
Maintain physical and web-based client files.
Establish and maintain therapeutic relationships with clients and families.
Facilitate and/or co-facilitate group counseling and recreation sessions.
Provide crisis intervention for clients and their families.
Completing thorough documentation in SWK database system.
Ensure clients participate or are offered all components or services offered at the program.
Provide advocacy specified by Southwest Key Programs and contract guidelines.
