Provide spiritual support to an average caseload of 45 Hospice patients and their families in Care Facilities and patient's homes.
Conduct spiritual needs assessments of patients and families offering care in collaboration with the RN and social worker.
Assist patients and families in communicating with community spiritual resources, including their clergy, coordinating receipt of sacraments, rituals, rights, and other needs.
Assist patients and their families in identifying, clarifying, and understanding their spiritual perspectives and root beliefs in order to use their personal strengths in supporting one another through the dying process.
Be present with dying patients and their families. Offer bedside prayers, ritual, and support prior to and following patients deaths.
Attend all hospice interdisciplinary group meetings and patient care conferences to provide consultation, education, and support.
Communicate with team members in a timely manor, in both written and oral form, for planning, implementing, and evaluating patient/family care.
Maintain up to date records of patient and family visits for proper scheduling purposes.
Assist with employee and volunteer orientation training and act as a resource for consultation in patient/family care.

Provide clergy support for area funeral homes, conducting services for those who have no church affiliation and would like Christian memorial/funeral services.
