I am a caring Program Coordinator/Supervisor that is excited about impacting group homes/programs and individuals I serve by providing positive support. I am familiar with person centered planning and always looking for ways to personalize assistance. I am well-organized and with a conscientious approach.
Was responsible for a team of, at times, up to 10 individuals receiving in-home services. Responsible for: helping individuals develop, create and complete their goals. Assisting in helping each person develop social skills by finding appropriate activities for them to do in the community. Facilitating their well being by setting up doctor/dental and other appointments and providing transportation to these. Assisting with medication set up and administration when needed. Setting up bi-annual meetings with each individuals team and if needed assisting persons served in attending these meetings. Responsible for daily charting and logging in program goals data sheets, case notes, medical notes. Monthly program data compilation. When necessary getting doctors/dentist referrals completed. Completing schedules to be sure each individual was getting the time they required when they required it. Trained and supervised staff. Created staff schedules. Prepared for annual and semi-annual client team meetings by updating health reviews, health needs forms, assesing program data and updating paperwork to report on client progress, and updating and preparing other forms needed for meetings by the program director. Attending individuals teams meetings. Keeping staff and self up to date on necessary training.
This program/home was set up for an autistic individual that is/was rehabilitating from being institutionalized and recovering from past traumas that occurred due to misunderstandings that ended with the individual becoming very physically aggressive. Annual training on how to deal with physically aggressive behavior, preferably through deescalation, was required to work at this program. Other required annual training required was medication administration and First aid/CPR. Monthly training on person centered planning and dealing with physical aggression was also provided at staff meetings. To be prepared to work with this individual the original staff involved in opening this site also had extensive training on autism and person centered planning. This was by far the most fun home that i have worked with purely due to watching the individual grow and progress.
