Dear Ms. Sampson,
It is with regret that I must inform you that I will be severing our therapy arrangements effective immediately. This was a decision made after much consideration and due to your own inability to maintain your appointment schedule.
When you first came to me, I was required to keep your physician and your insurance company up to date on your progress. After the first two months, everything was going fine and you seemed to be making progress. But in the past three months, you have only kept one appointment and I am getting pressured by the insurance company to severe our relationship.
I do not know if the insurance company is still compensating you for your injuries and that is why they are pressuring me to severe this relationship, but I am confident that it is none of my business. My practice can no longer afford to take the kind of scrutiny it has received lately and that is why I am severing the relationship immediately.
