Dear Mr. Mota,
I just wanted to take a minute to apologize for my poor performance at work lately. I know that I have been coming in late and not completing my best work while on the clock. For this I am truly sorry and I take full responsibility.
About three weeks ago my son was involved in an awful car crash and he had to be hospitalized for several days. While he is doing better now things have been very stressful between dealing with the doctors the insurance company and other family members. Now that my son has settled back in at home thankfully things have begun to calm down. We have the help that we need for him while I am at work and I am ready to give my attention back to my job again. I promise to do only my best work from this point forward.
Thank you very much for your understanding during this difficult time. I really appreciate your leniency over the past couple of weeks and I promise that you will not regret it. Please accept my most sincere apologies for my decreased performance during this time.
