As a medical scribe I recorded patient data and medical records for various ER physicians. I worked at 5 hospitals in rural and urban settings. Shifts correlated with the emergency doctors, so that every doctor had a scribe on shift. I coded disease and injury diagnoses, along with procedures in an inpatient setting. I also charted an extensive history, description of present illnesses, review of systems, lab values, radiology readings, and always included a disposition and diagnosis as dictated by the physician.
