Contracted out to the York Housing Authority to perform liaison work between public and private local community service organizations and residents of an affordable housing facility called Village Court. Duties were as followed: Met with clients, families and others to develop and maintain a working relationship and establish a treatment schedule, provided pre-vocational training, provided job development, job placement, job coaching, provided independent living skills training, provided training to develop community integration skills, completed and submitted treatment reports such as progress notes, monthly summaries, community based work assessments, job contact logs, completed and submitted administrative documentation such as productivity reports, encounter forms, mileage and work expense forms, communicated with the funding sources, employers, and other support services, attended and lead discussion of services and issues at team conferences, worked with a variety of clients in terms of age, gender, race, and disability
Assisted in all sides of the placement process of individuals and families that were homeless. Interviewed clients for eligibility, acted as placement coordinator, designed and maintain an efficient data management system, created and trained for a new position within the unit, as well as phones, filling, and payments to vendors.
Worked with the activity director in planning and participating in activities with the elderly and disabled population.
