Registered NurseREGISTERED NURSE, DIABETES EDUCATOR, Riverside Diabetes Wellness Center － Bourbonnais, IL
I am the -Diabetes Educator for Riverside's outpatient diabetes wellness center.
I build and establish relationships with patients to help facilitate their Diabetes Self-Management Education (DSME) through motivational interviewing, positive reinforcement, and assistance with problem solving.
I work with a team of phenomenal dietitians to provide DSME and Medical Nutrition Therapy in group and one-one settings.
Aside from my duties as an educator, I have successfully developed, planned and implemented a marketing plan with strategies to increase volume and physician referrals.

PROGRAM ADMINISTRATORREGISTERED NURSE Jackson Park Hospital － Chicago, IL
I oversaw the planning, implementation, and evaluation of the Diabetes Self-Management Education (DSME) program and managed the resident physicians during diabetes support sessions.
Working very closely with the staff dietitian allowed me to reinforce and encourage patients to make healthy food choices and follow their prescribed meal plan.
I provided DSME to patients who come to our outpatient clinic, and assessed, managed and administered care to patients admitted to the ICU or Medical-surgical units.
I also performed tasks to identify patients on all inpatient units with diagnosis that would qualify them for DSME once they were discharged.
JPH serves an extraordinary community of people who respond very well to those who genuinely want to help them.
I enjoyed the interactions I had with my patients and learned quite a bit from those that have attended my classes as well as those I have had the pleasure of meeting in passing.

RENAL NURSE CASE MANAGERMiki Lee Kidney Resource Services, United Healthcare/OPTUM － Lisle, IL
08/2006 to 08/2008
Nurse Case ManagerMEDICAL-SURGICAL NURSE, Medical Staffing Network / Unity Hospital － Muscatine, IA
in Kidney Resource Services (KRS), I identified, coordinated, and provided appropriate levels of care under the direct supervision of a RN Case Manager and the program Medical Director.
I was responsible for clinical operations and medical management activities across the continuum of care (assessing, planning, implementing, coordinating, monitoring, and evaluating).
This included but was not limited to case management coordination of care, and medical management consulting.
I provided health education and coaching to members as well as treatment decision support.
In this role I was responsible for contacting members and determining eligibility for KRS, placing outbound Welcome Home calls when members were discharged from an inpatient setting, and completed pre-service authorizations for dialysis services.
I established a working knowledge of the program's key features and benefits which allowed me to effectively communicate how the program was beneficial to eligible members.
I consistently documented and maintained call details within medical charts and provided information to the RN Case Manager or Medical Director for on-going follow-up.
I am able to manage cases for members who have commercial, Medicare or retirement plans and discussed details of their health plans effectively so they understood what is included in their designations.
My call audits were consistently between 94-100%.
As a medical-surgical nurse I was responsible for managing the care of adult patients who were experiencing general medical conditions or surgical procedures requiring assessments, therapies, and interventions related to their specific conditions.
I conducted ongoing, individualized patient assessments and prioritized the data I collected to develop indi administered care/treatments based on.
