Associate Advisory Committee - Interdisciplinary team including clinical employees, ancillary employees and administration. Discussed ways to increase employee job satisfaction and decrease employee turnover rate.
NPSG 3E Anticoagulation Committee - Responsible for updating current anticoagulation policy to include new anticoagulants and to reflect changes required by computerized physician order entry. Also writing CPOE order sets for all anticoagulants. Aided Clinical Effectiveness department in implementing new CMS Anticoagulation Core Measure requirements by creating Anticoagulation Core Measure computer documentation tool, assisting in education of nursing and physician staff,updating Anticoagulation Risk Score Assessment tool for nursing. Also,updating Anticoagulation Education for patients to include all parenteral and new oral anticoagulants.
Medication Management Committee - The committee responsibility is to review and assess compliance with Joint Commission Medication Management Standards. Pharmacist representative on subcommittee implementing new discharge medication reconciliation process. Helped with education of nurses, physicians and other pharmacists. Assisting in medication safety by monitoring for safe storage of medications, monitoring for appropriate expiration date documentation on opened vials, and reporting issues to the committee via completion of tracers.
FEMA Daily Dose Committee - This committee implemented several processes to ensure that patients receive home medications on day of admission. Committee began after several negative patient outcomes due to patient not receiving beta blockers, anticoagulants and other required daily medications on day of admission. Responsible for collecting PI data.
Medication Administration Committee - Pharmacist representative on committee convened to decrease the number of medication administration times. Committee was results of numerous requests from nursing for fewer times when medications had to be administered to assist in timeliness of doses and decreased workload.
CMS VTE Core Measure - Working with the Clinical Effectiveness department and the Anticoagulation Safety Committee to incorporate the new CMS VTE core measure guidelines into our current policies and protocols. Also determining ways to integrate VTE core measure guidelines with SCIP core measure guidelines.
Transition Pharmacist Pilot Committee - Assisted with expansion of pharmacist role in the Emergency Department. Helped to determine pharmacist responsibilities, most productive hours for staffing and equipment required. Assisted with education of entire pharmacy staff. Created emergency box with essential medications required for effective emergency response. Pharmacist responsibilities include responding to all emergencies: Condition Blue, Rapid Response, Stroke Alert, Chest Pain Alert, Trauma Alert. Update medication reconciliation list for patients being admitted for inpatient care via patient interview, retail pharmacy interview, physician office medication list. Provide patient education for prescribed medications prior to patient discharge from ED.
Crash Cart Updates -Assumed responsibility for updating contents on adult and pediatric crash carts. Assembled committee consisting of ED nurses, pediatric nurses, newborn nursery nurses, pharmacists and education department to review decades old list. Updated contents to reflect most recent ACLS and PALS recommendations. Submitted new recommended contents to all appropriate committees for approval. Worked with all pharmacy staff to update contents of 22 adult crash carts and 8 pediatric crash carts. Also updated Emergency Response policy to reflect new crash cart contents and AHA recommendations.
HIV PEP protocol update - Updated HIV PEP protocol to reflect recommendations of NIH and CDC. Also reviewed with Infectious Disease physicians and infection control department. Submitted to appropriate committees for approval.
Pulmonary Embolism Clinical Pathway - Currently working with interventional cardiologist to develop a clinical pathway for appropriate therapies for pulmonary embolism based on severity of condition. Developing algorithm for clinical pathway, alteplase full dose and half dose protocols, and protocol for appropriate diagnostic lab and imaging orders. Goal is to disseminate the clinical pathway to other area hospital emergency departments which regularly transfer patients to EMRMC.
Pyxis Replacement Project - Assigned to replace 55 Pyxis machines at two locations in thirty days. Working with one technician and a Carefusion analyst, the team has coordinated to accurately load and quickly exchange the old machines with new machines, while minimizing the impact on patient care.
Code Blue Committee - Review code blue responses on a monthly basis. Make recommendations for process improvements and training.
Pharmacy student preceptor - Assisted in furthering the education of pharmacy students from the University of Kentucky and Sullivan University. Preceptor for both IPPE and APPE rotations. Helped to develop critical thinking skills and clinical judgment, ensuring the strength of the next generation of pharmacists.
PGY1 Residency Preceptor for Internal Medicine Rotation - Assisting in resident education through educational presentations, cost effectiveness analyses, journal article reviews, and patient presentations. Participate in physician rounding with resident, as well educate and assist with clinical responsibilities on Internal Medicine floor.
